Registration

Please use a separate form for each student. If signing up for multiple classes please list each class separately.

Fill out name, address, city, state, zip code, phone, email and date of birth.

Annual $20 registration fee is required for first registration.

If you take more than one class you receive a 20% discount on the other class or classes. Also if more than one family member signs up for classes the other family member/s receive 20% discounts.

If a student refers a friend to Hunakai they receive a discount on their own classes.

Payments can be made in check, cash, credit card or an online payment with credit card or paypal. For checks,please fill out to Hunakai Studio of Fine Arts. In the memo please also include the class/es you will be taking.
For credit cards, the students may complete online or come to the studio to process.

Registration form and payment can be mailed to the studio’s PO Box.
